Resurrection
There are two ways out of the Fugue Plane: the Guardian who stands there, or another player bringing you back. Every route but one costs experience, and none of them can cost you more than your level allows.
The most you can pay
Whatever route is used, the penalty is capped by your level, and the cap is what you pay if the route asks for more:

A hardcore character in the dying state always pays at least 1%, even below level 5.
Below level 40 you cannot lose a level. The XP is taken down to the threshold of the level you hold and no further; whatever is still owed becomes XP debt, and you earn no experience until it is paid off. The debt cannot grow past twice your level in thousands - 40,000 at level 20. At level 40 there is no level above to protect, so the percentage comes straight off.
The Guardian in the Fugue
Four ways to be sent back, all of them raising you at your faction's respawn point:

Being brought back by another player
Only a party member can resurrect you - anyone else is told to join your party first. Nobody in the Fugue can resurrect anyone: the Guardian's offers are for themselves alone.
You are asked before it happens. A window shows your level, your XP, the most that can be taken, your current and maximum XP debt, and exactly what this resurrection would cost in XP and in gold, and nothing is taken unless you accept.

The gold comes out of the dead character's purse, and out of their bank account if the purse is short. If neither covers it, nothing happens.
A True Resurrection Balm can only be used by a Cleric of 15, a Paladin of 21, a Blackguard of 30 or a Druid of 40. A Pure Balm has no class requirement, costs nothing at all, and can be used once every 16 hours; some carry a limited number of uses and crumble when the last is spent.
A Temple Cleric has to be a real one - level 17 in the Cleric class, nobody's henchman, and not hostile to you.
Raised from your body you stand up where the body is. Sent back by the Guardian, or by a balm used on you from elsewhere, you appear at your faction's respawn point.
Experience debt
You do not lose a level to a resurrection. If the experience a raise costs you would drop you below your current level, your experience is set to the floor of that level instead and the rest becomes debt.
- While you owe debt you earn no experience. Everything you kill, every quest and every dungeon pays the debt down instead, until it is clear.
- Dying again while in debt adds to it, up to the limit - and once you are at the limit, a further death costs you no more experience at all.
- The limit is character level x 2,000. A level 39 character can owe at most 78,000.
Level 40 is the exception, and the only one: at the cap a resurrection can cost you a level or more, depending on the option you take.
When it is free
- A PvP kill that did not meet the server's requirements for one.
- A free resurrection event, which covers every death outside a dungeon.
- A DM.
- A Pure Balm, always.
A free resurrection is also the only kind that saves a hardcore character in the dying state, and only within ten minutes of the death.
